Kindly fill in all the details requested for and get the same printed out. Affix signature of the appropriate authority and send/submit the hard copy of this along with the samples and dd to us. You will be receiving a copy of the form to your e-mail id. A temp id will be provided. In case samples and the signed hard copy and the dd are not received within one month the request will be deleted from our data base. You may have to re enter and submit the form. After we receive the samples, dd and the form we shall register your work order and shall send you the work order number. In case you do not receive the work order number you may contact the SAIF office indicating the temporary id received. Your suggestions to improve the system will be deeply appreciated. |
